Carefirst is a unique, first of its kind, culturally sensitive and inclusive, community-based health and wellness organization serving a growing population of Chinese and South East Asian seniors in the Greater Toronto Area.
The new, 53,000 ft2 facility is found on the PACE Model of Care; its design facilitates the integration, coordination, and delivery of preventative, primary, acute and long-term care services, consolidating several existing facilities in the GTA into one holistic community care hub. This care hub supports a family health team, a 30-bed short-stay transition care program, an adult day program, mental health counselling, wellness coaching, social services, chronic disease management (diabetes, dialysis treatment, etc.), vocational training and health education.
Mindful of the values that define Chinese and South East Asian cultures, the design draws on the planning principles of a traditional Chinese courtyard house to offer the seniors it serves a care environment with familiar, comforting features. The lush garden, in particular, placed at the heart of the building, provides the Carefirst community with a contemplative retreat, place of exercise and amenity for gardening.
This industry-leading initiative assists clients in continuing to live safely, healthily and independently and has been identified by the University of Toronto’s Health Management Institute as an exemplary provider of integrated care services.
